[Chart] upcoming minor release

Hi,

There's a collection of pending minor changes and bug fixes as per below 
(thanks to those who submitted them). I'd like to release these as v 1.2.1.

Let me know in the next day or so if there's anything else you think 
should be fixed or included.

     The test reference images are out of date, and depend on external
     libraries (eg cairo and diagrams) for pixel accuracy. So, I've
     removed them - the existing harnesses can be used to generate a
     set of test images as required.

     We can't upgrade lens until the diagrams library does so.
